ESSILOR LENS AUTHENTICATION

The insured shall immediately notify the loss or damage to the dealer / retail outlet of the lens.
If the loss is due to burglary, theft, immediate complaint should be made to the police.
If the lens is broken or damaged, the same is to be deposited with the dealer / Retail outlet against acknowledgement.
Depreciation rate @8.33% per month is applicable from the date of purchase.
The following documents are to be submitted to the dealer / Retail outlet against the acknowledgement
1. Certificate of Insurance duly stamped by the retail outlet
2. Claim form
3. Original Invoice
4. Authenticity card
5. Broken lens ( In case of breakage / accident)
6. Police record (In case of theft or burglary)
7. Maximum limit is Rs.25,000/- only
